Welcome to the Quality Improvement Indicator Data Collection (QIDC). It runs online or can be provided to your IT staff for internal only access. The QIDC allows clients to capture data associated with our Quality Improvement Indicators (QII) and Quality Improvement Programs (QIP). This includes:

Medical Dental Eye Care Offices
All Animal Facilities (Veterinary Facilities, Animal Hospitals, Animal Labs, Animal Rescue, Kennels, Zoos, etc.)
Assisted Living, Rehabilitation Facilities, and Nursing
Hospitals
Schools
Everyone Else

Sample Report - Everyone Else

This is similar to the Schools example. For small facilities there may be just 3-10 rooms. For large facilities there can be hundreds of rooms like in the school example. The room names change to match the facility.
